Safety Management System- draft Incident Report Investigation template

In any investigation it is important to gather the facts surrounding the incident, the parties involved, and the action taken by them.
From that we can determine the appropriateness of the response, and any remedial action required to mitigate any future occurrence.
The template below is a draft outline for such an investigation and report.
It will certainly change and expand on a case-by-case basis depending on the nature of the incident and investigation.


Pilot/owner

  • The incident- when, where, how and what happened?
  • The damage- what, where, extent, photos?
  • Action taken- who accident reported to, who consulted re appropriate repairs, who undertook the repairs?
  • Reason for inspection- repair, return to service or routine annual?
  • Information provided to IA- full disclosure of the incident, damage and repairs made?
  • Declaration on annual inspection form?
  • Awareness of responsibility to report accident and advise IA?

IA

  • Reason for inspection- repair, return to service or routine annual?
  • Information provided by owner- full disclosure of the incident, damage and repairs made?
  • Action taken- who consulted re appropriate repairs/checks, who undertook the repairs/checks, results?
  • Awareness of manufacturer’s requirements, best practice for this type of incident?
  • Advice given to pilot/owner?

Analysis

  • Was the incident reporting satisfactory?
  • Was the repair satisfactory?
  • Was the inspection satisfactory?
  • Any failings, deficiencies on pilot/owner’s part?
  • Any failings, deficiencies on IA’s part?
  • Any contributing issues with RAANZ information, systems, documents, advice to pilot/owner or IA?

Recommendations

  • Airworthiness of the aircraft?
  • Remedial action on pilot or pilot/owners in general?
  • Remedial action on IA or IAs in general?
  • Remedial action on RAANZ processes, systems, documentation?
